Terms
|
I understand that participation in dance activities involves the risk of injury and enroll the above named person at his/her/my own risk. I also agree to inform the School of any physical or mental conditions that the staff should be aware of. I understand that A.S.I.T.R.D. Dance School and its owners and employees, is/are not responsible for any personal belongings left or lost in the Dance School .

Tuition
Policies
|
The monthly tuition is $47.00 per student for a single class, $38.00 each additional class, $54.00 for one and a half hour classes, $23.00 for half hour class offering; not including solo/duet/trio class and $65.00 for Solo lessons. Tuition IS NOT prorated. All dancers are entitled to make-up class credits. Should extenuating circumstance arise, you are to contact studio owners Tara and Michael Mahoney, and your situation will be given personal attention. A $20.00 non-refundable registration fee per student is charged before October 1st and $30.00 after this date.

Payment
|
Bills are not routinely sent. TUITION PAYMENT IS DUE THE 15TH OF THE MONTH FOR THE UPCOMING MONTH ex. August 15th for September, September 15th for October and so on. A late fee of $15.00 will be charged on any account not paid In full bv the due date. RETURNED CHECKS WILL BE charged a $25.00 banking fee. A $5.00 processing fee will be charged to your account for each payment submitted without account coupon.

Missed
Classes
|
Since your class time is reserved, and in order to preserve a low student/teacher ratio, it is understood and agreed that no refunds or credits are to be given for missed classes. It is also understood and agreed that the participating student or a responsible parent or guardian will notify the School PRIOR to any unavoidable class absence in order to be eligible for a make-up class. Unless this is done a missed class will be considered to have been taken, and no make-up will be offered. Otherwise, make ups will be offered at the discretion of the staff.

Cancel
|
Notice of intent to withdraw from any class must be given to the desk 24 hours before the first monthly class meeting. By signing this contract I signify that I understand and agree to all terms and conditions herein, and agree that I am responsible for payment of all classes reserved for the above named enrollee up to the date of cancellation. Notice of cancellation must be given to the office staff by an adult. Student cancellation for temporary extended periods of absence will not relieve the participating student from the requirement of making all monthly payments of class fees.
